COMET voor een negatief zelfbeeld ...

Samenvatting

Een negatief zelfbeeld lijkt van invloed op het ontstaan van en terugval in depressie. Tot nu toe zijn er weinig specifieke interventies voor het verbeteren van een negatief zelfbeeld beschreven en onderzocht. Competitive Memory Training (COMET) voor negatief zelfbeeld is gebaseerd op inzichten en bevindingen uit de experimentele psychologie. In aansluiting op bevindingen bij andere populaties is het onderhavige onderzoek een pilotstudie naar het effect van COMET voor negatief zelfbeeld bij ambulante patiënten met een depressieve stoornis en een negatief zelfbeeld. De COMET-training vond plaats naast de reguliere behandeling. Tijdens COMET + reguliere therapie namen zelfwaardering en autonomie significant en in belangrijke mate toe, terwijl depressiviteit op vergelijkbare wijze afnam. Deze verbeteringen traden in ongeveer gelijke mate op bij patiënten met verschillende depressiediagnoses. Post hoc werd een convenience vergelijkingsgroep toegevoegd van patiënten die alleen reguliere behandeling voor hun depressie hadden gehad. Vergeleken met de vergelijkingsgroep verbeterden het zelfbeeld en de autonomie van de mensen in de COMET + reguliere therapieconditie significant en in belangrijke mate. Daarnaast namen de depressieve klachten significant en in belangrijke mate af. COMET voor negatief zelfbeeld lijkt een effectieve interventie bij depressieve patiënten met een negatief zelfbeeld. Tekortkomingen en klinische implicaties van dit onderzoek worden besproken.

Summary

Low self-esteem seems to be an important factor in the development of and relapse in depressive disorders. To date, little specific interventions that aim at improving low self-esteem have been described or researched. Competitive Memory Training (COMET) for low self-esteem is based on insights and findings from experimental psychology. In addition to studies into other populations, the present study is a pilot study investigating the effect of COMET in depressed patients with low self-esteem in a routine mental health setting in addition to regular treatment. During COMET + regular treatment, self-esteem and autonomy increased significantly and in a large degree, while depressive symptoms decreased to a similar extent. These effects occurred in a roughly similar degree in patients with different depression diagnoses. Post hoc, a convenience comparison group of patients who had only had regular treatment for their depression was added to the study. Compared to the control group, self-esteem and autonomy of the patients in the COMET + regular treatment condition improved significantly and largely. Also, a significant decrease in depressive symptoms was found. COMET for low self-esteem seems to be an effective intervention for depressed patients with low self-esteem. Shortcomings and clinical implications of this study are discussed.
