Samenvatting

Hoewel verschillende methoden effectief zijn gebleken in de behandeling van gegeneraliseerde angststoornis (GAS), blijft de effectiviteit ervan bij de meeste andere angststoornissen een stuk minder. Mogelijk wordt dit in de hand gewerkt door het feit dat er betrekkelijk weinig interventies zijn ontwikkeld die zich specifiek richten op de meest karakteristieke kenmerken van de stoornis. Nu piekeren wordt beschouwd als dat centrale kenmerk van GAS, zijn er inderdaad enkele nieuwe veelbelovende specifieke methoden ontwikkeld, zoals metacognitieve therapie en een methode die zich speciaal richt op het reduceren van het onvermogen om met onzekerheid om te gaan. Deze nieuwe methoden moeten echter nog grondig worden getest op hun waarde. Een andere nieuwe methode is competitive memory training (COMET) voor piekeren en rumineren. Na een beschrijving van deze methode wordt een eerste baselinegecontroleerde pilotstudie gepresenteerd. Patiënten (35) kregen zes groepszittingen COMET, terwijl hun reguliere behandeling doorging. Zij vulden drie weken voor de start van COMET vragenlijsten in (M1), bij de start van COMET (M2) en aan het einde ervan (M3). In een ‘intention to treat-analyse’ waarbij ontbrekende scores waren geïmputeerd volgens de ‘last observation carried forward-procedure’, bleken er tijdens de baseline geen significante verbeteringen te zijn opgetreden, terwijl dit tijdens COMET wel gebeurde op alle uitkomstvariabelen. Beperkingen van de studie en mogelijke implicaties van de resultaten worden besproken.

Summary

While several methods have been proven to be effective in the treatment of generalized anxiety disorder (GAD), efficacy is still worse than in most other anxiety disorders. It is suggested that this state of affairs is caused by the relative paucity of treatment methods specifically aimed at the most identifying features of GAD. Since worry is considered to be the most identifying aspect of the disorder, several promising new treatment methods targeting specifically the worry component have been developed. One of these new methods is Competitive Memory Training (COMET) for worrying and rumination. After a short introduction with a case illustration, a first baseline controlled pilot study into the effectiveness of this method is presented. 35 Patients received six sessions of COMET in groups, while their regular therapy continued. They filled in questionnaires three weeks before the start of COMET (M1), at the start of COMET (M2) and at the end of it (M3). In an intention to treat analysis in which missing values were substituted by the last observation carried forward (LOCF) procedure, no changes during baseline could be detected, while during COMET significant progress was realized at all outcome variables. Limitations of the study and possible implications of the results are discussed.
