COMET: de behandeling van obsessies met contraconditionering ...

Samenvatting

Er wordt een experimentele methode beschreven voor het behandelen van obsessies. In Competitive Memory Training (COMET) leren patiënten met behulp van een uitgewerkt stappenplan voelen wat ze al weten, namelijk dat hun obsessies niets meer zijn dan rare, maar verder betekenisloze gedachten. De theoretische achtergronden van COMET worden uiteengezet en het protocol wordt beschreven. De toepassing van COMET wordt geïllustreerd aan de hand van enkele behandelingen. In de discussie wordt onder meer gesuggereerd dat COMET met name een goede aanvulling op exposure met responspreventie zou kunnen zijn voor het behandelen van obsessies met een vooral ‘referentieel karakter’. Dat wil zeggen dat de obsessie niet zozeer een concrete negatieve gebeurtenis voorspelt, maar dat deze veeleer een intrinsieke negatieve betekenis genereert.

Summary

An experimental method for treating obsessions is described. In Competitive Memory Training (COMET) patients learn to feel what they already know, i.e. that their obsessions are nothing but weird though meaningless thoughts. The theoretical background of COMET is clarified and the protocol is fully described. The practice of COMET is illustrated by three cases. In the final discussion it is suggested that COMET might be a welcome addition to exposure with response prevention in the treatment of obsessions of a ‘referential nature’. This implies that the obsession does not predict a real uncoming negative event, but rather that it generates a negative connotation.
